EMDR vs TMS: psychotherapy or brain stimulation

EMDR is psychotherapy; repetitive transcranial magnetic stimulation (rTMS) is a noninvasive medical procedure using magnetic pulses. Both may appear in searches about trauma, depression or OCD, but they are not versions of the same treatment.

What treatment is like

rTMS applies pulses through a coil placed on the scalp, commonly in repeated sessions over several weeks. It should be prescribed and monitored by a medically trained team with relevant expertise.

NIMH summarizes US rTMS authorizations for treatment-resistant depression, OCD and other indications; regulation and availability vary by country and device. Authorization should not be generalized to every symptom.

For PTSD, NICE recommends EMDR and trauma-focused therapies. TMS reviews show active PTSD research, but protocols, brain targets and certainty remain variable.

Risk and burden

EMDR may cause temporary distress, vivid dreams or activation and needs a stabilization plan. TMS may cause scalp discomfort, muscle contractions, headache or dizziness; seizure risk is low but present.

Compare total time, frequency, cost, medical contraindications, medication, access and follow-up. Do not stop prescribed treatment without consulting the prescriber.

Choosing or combining

Someone may receive rTMS for depression and psychotherapy for trauma, but combination requires clear goals and coordination. Different mechanisms do not prove that using both is superior.

Ask which diagnosis is targeted, what authorization and evidence apply to the device or protocol, who monitors adverse effects and how functioning—not only symptom scores—will be assessed.
