EMDR vs talk therapy: differences and how to choose

“Talk therapy” covers very different approaches: supportive therapy, psychodynamic therapy, counselling, CBT and others. Comparing it with EMDR requires knowing the specific treatment being offered.

Short answer

EMDR follows a phased protocol and, when used for PTSD, directly addresses memories and triggers. A mainly conversational therapy may focus on insight, support, patterns or skills depending on its model. No label determines fit alone: diagnosis or formulation, evidence, practitioner, alliance and preference all belong in the decision.

It is not a comparison between talking and not talking

EMDR also uses conversation for assessment, planning, preparation and review. What often changes is how much narrative occurs during processing and how attention to a memory is structured.

Some talk therapies are manualized trauma-focused treatments, while others are open-ended or supportive. Always ask for the exact model and plan.

Goal and evidence

For PTSD, guidelines including NICE and VA/DoD recommend trauma-focused therapies such as EMDR and specific cognitive-behavioural protocols. Talking about an event without a defined protocol is not automatically equivalent.

For a non-traumatic problem, another therapy may have stronger evidence. The presence of a difficult experience does not make EMDR a universal first choice.

The therapeutic relationship matters too

Feeling understood, being able to disagree, understanding the process and retaining control support work in any modality. A good alliance does not replace technical competence, and a credential does not replace relational safety.

You can ask how progress will be reviewed and what changes if the method does not help.

Choosing without false either-or choices

Someone may begin with assessment and support, move into focused work and return to integration. Some therapists combine methods, but should explain each component, their training and its purpose.

You do not need to leave helpful therapy simply because another approach is marketed as faster. Coordinate transitions to avoid conflicting plans or duplicated care.
