EMDR, suicidal thoughts and self-harm: evidence and safety

Short answer

Preliminary research has studied EMDR within treatment for adults reporting suicidal ideation, but it does not establish EMDR as crisis care or a first-line intervention for preventing attempts. Immediate safety, psychosocial assessment, crisis planning and coordinated care come first.

If risk is immediate

If you may harm yourself now, have a plan or cannot stay safe, call local emergency services or attend an emergency department. In Spain call 112 or the 024 support line.

Do not remain alone when danger is immediate. Reduce access to means where possible and ask a trusted person to stay with you while help arrives.

What EMDR research shows

A 2024 randomized trial included 42 adult outpatients with suicidal ideation and compared online EMDR added to usual care with usual care. Findings were promising, but the sample was too small for a conclusive between-group comparison.

Other studies are small or concern co-occurring depression or trauma. This does not make EMDR a substitute for interventions with stronger evidence for preventing self-harm or attempts.

What should happen before reprocessing

Assessment needs to explore intent, plans, access to means, previous episodes, substance use, support, protective factors and recent changes rather than relying on one risk score.

A collaborative safety plan may list warning signs, coping steps, people and services to contact and ways to reduce access to means. The therapist should explain when processing would stop and additional help would be activated.

EMDR as one part, not the whole plan

When traumatic memories contribute to distress, a team may consider EMDR once adequate safety and follow-up are in place. Other psychological treatments, medication for co-occurring conditions, social care or more intensive support may take priority.

Do not hide worsening suicidal thoughts because you fear therapy will stop. Sharing them allows support to be adjusted and decisions to be made together.
