EMDR, psychosis and schizophrenia: evidence and safety

Psychosis does not automatically rule out trauma treatment, but EMDR is not a replacement for comprehensive psychosis care. The clinical question is usually whether it can address PTSD or traumatic memories within a specialist plan.

Trials and a 2025 review suggest EMDR may reduce PTSD symptoms for some people with co-occurring psychosis without a clear increase in serious therapy-related events. Evidence remains limited and does not show that EMDR alone treats schizophrenia or an acute psychotic crisis.

What is actually being treated

The most relevant studies usually include people with psychosis and PTSD or post-traumatic symptoms. Trauma is the main target, although some studies also measure paranoia, negative symptoms or functioning.

Associated improvement should not be confused with a claim that EMDR is a stand-alone treatment for hallucinations, delusions, disorganization or psychotic relapse.

What recent research found

A 2025 systematic review included four randomized trials and reported improvement in PTSD and some psychosis dimensions, with no serious adverse events attributed to treatment. The authors called for larger trials and longer follow-up.

A 24-person forensic-setting trial found a favorable difference in PTSD symptoms at six months. Its size and setting limit generalization.

Assessment, pacing and the wider team

Stability, meaning of experiences, risk, sleep, substance use, medication, support and ability to remain oriented should be reviewed before starting. The plan may require adapted sessions and frequent reevaluation.

Coordination with psychiatry or a community team is particularly important after medication changes, recent relapse, admission, increased risk or rapid deterioration.

When urgent help is needed

A rapid loss of contact with reality, severe agitation, inability to care for oneself or danger to self or others needs urgent assessment—not routine memory processing.

A competent therapist explains which signs would pause EMDR and how the team or emergency services would be contacted.

Does psychosis always rule out EMDR?

Not necessarily, but it requires individual assessment, relevant expertise and coordination. The target and current stability matter.

Can EMDR cure schizophrenia?

There is no basis for that claim. Research mainly concerns co-occurring trauma or PTSD within broader care.

Can EMDR be used during an acute psychotic crisis?

An acute crisis needs urgent assessment and stabilization. Routine reprocessing is generally not the priority at that point.
