EMDR for fibromyalgia: evidence and limitations

Fibromyalgia is a real condition involving pain, fatigue, sleep and cognitive difficulties that may profoundly affect life. Exploring psychological factors or trauma never means that pain is imaginary or caused by insufficient effort.

Short answer

A randomized trial and a chronic-pain review show potential signals for EMDR, but evidence does not yet make it standard fibromyalgia treatment. Care should remain integrated and coordinated; NICE considers exercise, ACT or CBT for chronic primary pain according to circumstances and preference.

Integrated pain assessment

Assessment considers medical diagnosis, sleep, activity, fatigue, medication, mood, trauma, work and goals. Chronic pain may coexist with PTSD without either fully explaining the other.

Changing symptoms, neurological deficits, fever or another medical concern need appropriate healthcare. Psychotherapy should not stop indicated investigation or invalidate illness.

Supported treatment options

NICE recommends person-centered assessment and considers supervised exercise, ACT or CBT for chronic primary pain. EULAR also emphasizes education, exercise and individualized non-drug management.

These recommendations are not one prescription for every fibromyalgia case. Medication, physiotherapy, sleep and other interventions depend on assessment and applicable guidance.

What we know about EMDR

A 2024 randomized trial reported improvement in fibromyalgia, pain, sleep, depression and trauma measures. A 2025 review of EMDR for chronic pain found potential alongside study differences and limitations.

Independent replication, active comparators and adequate follow-up are needed. EMDR may target co-occurring trauma, pain memories or related fear without promising to remove the medical condition.

Choosing responsibly

Look for medical coordination and pain experience alongside EMDR training. Ask whether the target is PTSD, pain impact, fear, sleep or functioning and how each outcome will be measured.

Avoid cure promises, “stored emotional pain” presented as diagnosis or pressure to abandon care. Psychological improvement is valuable even when pain remains.
