EMDR for BPD: evidence, co-occurring trauma and safety
Borderline personality disorder (BPD) does not automatically mean PTSD, although they may coexist. The clinical question is not only “does EMDR help?” but which problem is being treated, with which risks and within what plan.
EMDR is supported for PTSD, not as a universal treatment for every dimension of BPD. Trials published in 2025 offer promising findings in people with BPD or borderline symptoms, but they do not replace comprehensive guidance or establish EMDR as first-line treatment for BPD generally.
Separate the diagnosis from the treatment target
A plan may address co-occurring PTSD, traumatic memories, self-harm, crises, relationships, identity or emotion regulation. One method may not cover every target.
Assessment should include suicide and self-harm risk, substance use, dissociation, support, current treatment and the ability to maintain safety between appointments.
What recent studies add
A 2025 trial comparing EMDR with CBT for BPD reported improvement, but only 18 of 76 participants completed the study; that attrition sharply limits confidence and generalization.
Another 2025 trial in people with PTSD and at least four borderline symptoms compared EMDR alone with EMDR plus dialectical behaviour therapy. Both groups improved and PTSD outcomes did not differ significantly at one year; this answers a specific population and design, not all BPD.
What guidelines say
NICE guidance for BPD addresses structured, coordinated care, crisis management and treatment of co-occurring problems. It does not present EMDR as a general first-line treatment for BPD.
Where PTSD is present, its specific recommendations may be relevant, but they need integration with BPD-related needs and continuity of care.
What to look for in a therapist
Ask about genuine experience with BPD, PTSD, dissociation, self-harm and crises; how readiness for processing is decided; what support exists between sessions; and how they coordinate with psychiatry or other services.
Do not accept promises to “erase” BPD by processing one memory. For immediate danger or suicide risk, contact emergency services where you are.

Does EMDR cure BPD?
